About the Role

As the Nursing Care Coordinator Team Leader, you will take charge of the operational and clinical leadership of our national hospital substitution programs. You’ll guide a team of dedicated professionals to deliver care that aligns with the unique needs of our customers while driving outcomes that benefit all stakeholders.

This permanent full-time role is based at our West Perth Office, with the flexibility to work from home a few days a week.

 

The Day-to-Day

  • Lead a multi-disciplinary team that provides coordinated, patient-focused care
  • Drive clinical assessment, care planning, and service coordination to meet patient needs
  • Foster innovation in healthcare delivery and implement new ways of working
  • Oversee service integration with other health teams, ensuring seamless coordination
  • Ensure the team delivers on quality clinical care and exceptional customer service
  • Support the development of specialized clinicians and support staff
  • Work closely with stakeholders to ensure desired outcomes for all parties
