Our Opportunity to You
We are looking for a talented Clinical Care Coordinator (Registered Nurse) to join our Remedy family. Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of high-quality home-based healthcare services to clients across Australia across both our Hospital Care and Rehab at Home programs.
This is a 12-month Fixed-Term Full-Time opportunity. The role offers a hybrid working arrangement and is open to candidates based in West Perth, WA who can work from home and on-site as required.
The Day to Day
- Coordinating and managing Hospital Care and Rehab at Home programs for clients transitioning from hospital to home-based care, including those following orthopaedic surgery or experiencing deconditioning after hospital admission.
- Supporting and communicating with clients remotely to achieve their personal and clinical health and rehabilitation goals within their home environment.
- Implementing, monitoring and modifying care plans to meet individual client needs.
- Managing clinical case escalations and variations to expected clinical progress.
- Collaborating with multidisciplinary teams to ensure safe and effective care delivery.
- Building and maintaining relationships with key stakeholders including hospital referrers, specialists, surgeons, service providers and funders.
- Supporting clients to achieve positive health outcomes while remaining independent at home.
- Ensuring accurate clinical documentation and compliance with organisational and regulatory requirements.
- Helping to improve the health and wellbeing of hundreds of Australians.

About You
- Current registration as a Registered Nurse with AHPRA.
- Experience working within a multidisciplinary healthcare team.
- Strong clinical assessment, reasoning and problem-solving skills.
- Understanding of hospital discharge planning and community-based care.
- Excellent verbal and written communication skills.
- Strong organisational, administrative and time management skills.
- Comfortable using clinical systems and technology.
- Ability to work in a dynamic environment with tight timeframes and competing priorities.
- Ability to work autonomously while managing competing priorities.
- Passionate about improving health outcomes and making a meaningful difference in the community.
- Must be based in Western Australia.
