Your role 


Australian Unity is excited and proud to be partnering with Ramsay Connect to deliver the Diabetes Using Our Strengths Service (DUOSS) pilot research program. The DUOSS is an innovative virtual and face to face service that incorporates remote patient monitoring, optimising management of diabetes medicines, education, and group yarn ups to help Aboriginal and Torres Strait Islander peoples to manage their type 2 diabetes. 

 

As a Community Registered Nurse on this program, you will lead the clinical delivery of the Diabetes Using Our Strengths Service (DUOSS). The role will work in a digitally enabled model of care and be responsible for the delivery of quality, clinical care for patients in managing their type two diabetes in a holistic and culturally appropriate way. 

 

Role objectives will be achieved through effective and responsive chronic disease management working in a collaborative multi-disciplinary team including working closely with our Diabetes Educator and Aboriginal Health Worker.

 

This is a full-time 12 month fixed term role, with travel to Bundjalung Country required. The project is funded by MRFF funding from the Australian Government’s Targeted Translation Research Accelerator program, delivered by MTPConnect. 


About you 

You will deliver technology enabled, culturally appropriate care including: 

  • Face to Face and Remote patient monitoring
  • Evidence based chronic disease management
  • Care Coordination
  • Clinical escalation of any concerning results, signs, or symptoms
  • Health coaching

 

Your expertise and qualifications will include:

  • A demonstrated ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people and to facilitate culturally appropriate group yarning sessions
  • Registered Nurse with unrestricted AHPRA registration and community nursing experience 
  • Clinical skills in chronic disease management and delivering care within a multi-disciplinary framework

Desirable:

  • Experience in working in virtual care 
  • Experience in providing Diabetes Management 
  • Living on Bundjalung Country
