As the Head of Care Partnering, you will lead the establishment and growth of the Care Partnering function within your region. This strategic leadership role is focused on integrating services, optimising care coordination, and ensuring that each customer’s care journey is seamless and tailored to their individual needs. You will drive high-quality, person-centred care that empowers individuals to live well at home or in their chosen environment.

 

This is a full-time opportunity, ideally based in NSW, though we are open to candidates from Victoria or Queensland.

 

In this role, you will be responsible for:

  • Leading the Care Partnering function, ensuring seamless service integration and enhancing care coordination and service matching for improved health outcomes.
  • Driving operational excellence by optimizing care pathways, improving clinical processes, and fostering a culture of innovation and continuous improvement.
  • Proactively identifying and executing growth opportunities while maintaining a strong commercial focus, ensuring high standards of care, and maximizing service delivery.
  • Collaborating with internal stakeholders and external partners to ensure that the customer’s needs are met through a comprehensive needs assessment and care planning process.
  • Managing the development of a high-performance team, ensuring alignment with organizational values and fostering a customer-first approach across all services.

 

About you:

  • 10+ years of leadership experience, including experience in NDIS plan management, with a strong understanding of the NDIS funding model.
  • Proven expertise in the health industry, with a focus on driving scale, growth, and the successful delivery of health and care services.
  • Extensive experience with Aged Care and Community Health Quality Standards, accreditation frameworks, and a deep understanding of government and private aged care funding programs.
  • A strategic mindset with exceptional skills in stakeholder engagement, communication, and operational leadership.
