As a Care Partner Team Leader, with our Home Health team, you’ll guide, mentor, and uplift a passionate team of Care Partners. Your leadership will create a vibrant neighbourhood of connected health and community professionals who work together to support customers to live and age well at home. You’ll foster a culture where customers feel heard, supported, and confident in their care journey — and where your team is empowered to deliver safe, high-quality, person-centred care every day.

 

This is a full-time role based at our Perth WA branch and region.

 

Key Accountabilities

 

  • Lead, mentor, and support a team of Care Partners to build a positive, engaged, and high-performing culture.
  • Embed customer-centred practices through meaningful conversations, holistic assessments, and high-quality health planning.
  • Monitor performance, including customer experience, service levels, documentation quality and timely completion of health plans.
  • Guide your team to coordinate multidisciplinary care, navigate complex systems and connect customers to the right services when they need them.
  • Work collaboratively with Clinical Care Teams to manage escalations, support complex customer needs and ensure safe, high-quality outcomes.
  • Identify opportunities to enhance customer experience, support service growth and balance customer needs with operational priorities.
  • Lead and support continuous improvement, quality, safety, compliance and change initiatives.
  • Use enterprise systems to maintain accurate customer data and manage operational workflows.
  • Promote a safe, inclusive, and supportive working environment, ensuring adherence to clinical, risk, and legislative requirements.

 

What You’ll Bring

 

  • Experience leading teams in a fast-paced, customer-focused environment.
  • Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
  • Strong people leadership skills, with the ability to coach, support, and uplift team capability.
  • Excellent communication, problem solving, decision-making and time management skills.
  • Proficiency in Microsoft Office and familiarity with CRM systems.
  • Knowledge of aged care funding programs, quality standards, clinical incident processes and audit requirements.
  • Ability to work collaboratively in a matrix environment with clinical, operational, and external stakeholders.
  • Relevant tertiary qualifications (nursing, allied health, community care) desirable but not essential.
