Our Opportunity to you
We are looking for a talented Registered Nurse to join our Remedy family. Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of home-based health care to clients across Australia. 


 
This fixed term full-time (12 months) role is based at our Perth office, with the flexibility to work from home a few days a week.
 
The day to day 

  • Coordinating and managing acute Hospital Care at Home programs for clients discharging early from hospital to home-based care
  • Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment.
  • Implementing, monitoring and modifying in-home care plans to meet client needs
  • Managing clinical case escalations and variations to expected clinical progress
  • Supporting clients to achieve their personal and clinical goals within their home environment.
  • Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers and funders

About You

  • Qualification as a Registered Nurse with current AHPRA registration
  • Experience working in a multidisciplinary collaborative team
  • Excellent knowledge of hospital discharge planning
  • Ability to work in a dynamic environment with tight timeframes and competing priorities
  • Excellent clinical reasoning and problem-solving skills
  • Excellent verbal and written communications
  • Strong administration and time management skills, tech savvy
  • Passionate about health and committed to making a real difference in the community
