Our Opportunity to you

We are looking for talented Registered Nurse to join our Remedy family. Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of home-based health care to clients across Australia.

 

This hybrid role will be working out of our Perth Office and from home on a Permanent full time basis.

 

The day to day

  • Coordinating and managing acute nursing Hospital Care at Home programs for clients discharging early from hospital to home-based care
  • Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment.
  • Implementing, monitoring and modifying in-home care plans to meet client needs
  • Managing clinical case escalations and variations to expected clinical progress
  • Supporting clients to achieve their personal and clinical goals within their home environment.
  • Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers and funders
  • Helping to improve the health and wellbeing of hundreds of Australians

About You

  • Qualification as a Registered Nurse with current AHPRA registration
  • Experience with complex wound care and IV antibiotic management
  • Experience working in a multidisciplinary collaborative team
  • Excellent knowledge of hospital discharge planning
  • Ability to work in a dynamic environment with tight timeframes and competing priorities
  • Excellent clinical reasoning and problem-solving skills
  • Excellent verbal and written communications
  • Strong administration and time management skills, tech savvy
  • Passionate about health and committed to making a real difference in the community
