Lead and inspire a multidisciplinary team to deliver high-quality, person-centred care to our Aboriginal Home Health customers while shaping the Service Delivery function. Your Opportunity

 

The Neighbourhood Leader is a visionary and inspirational person eager to make a difference in the lives of ageing Aboriginal Australians. As the conductor and orchestrator of the neighbourhood ecosystem, the Neighbourhood Leader puts the customer front and centre, while coordinating the day-to-day operations of the neighbourhood to deliver safe and high-quality care to customers and patients. You will directly lead a team of Care Team Leaders whilst also being the driving force to inspire the broader neighbourhood team, Practice Leads, and multidisciplinary team (MDTs) to embrace new ways of working, always focused on customer outcomes.

 

As the Neighbourhood Leader, you will work alongside the broader ecosystem to strengthen the integration and delivery of services as well as working relationships. This role will bring together people from across our Home Health business, external health partners, and agencies, to develop a high-performing and cohesive team with the right values, behaviours, and competencies. This permanent full-time role is available within our Metro Sydney and surrounds location.

 

Your Skills and Experience

 

  • Certificate IV or Diploma in a relevant field, Business qualification (or equivalent experience)
  • Demonstrated operational and frontline experience in the care industry
  • Demonstrated experience leading geographically dispersed teams
  • Excellent verbal and written communication skills combined with the ability to engage with a frontline workforce using various communication channels
  • Proficiency using the Microsoft Office suite of applications
  • Working knowledge of regulatory requirements including funding types and aged care quality standards
  • Ability to build partnerships within the business and achieve outcomes through others
  • Experience working with Aboriginal Medical Services (AMS) is preferred
