As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.

 

This is a full-time role based at either our Mulgrave or Clayton Branch. 

 

Key Accountabilities

  • Build trusted relationships with customers and support them to achieve their goals at home.
  • Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
  • Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
  • Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
  • Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
  • Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
  • Promote safety, quality, compliance, and a culture of continuous improvement across all activities.

 

What You’ll Bring

  • Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
  • Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
  • Excellent verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
  • Knowledge of funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning.
  • Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
  • Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers.
  • Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
