We are seeking a passionate and client-focused Health Coach to join our growing team. In this role, you'll work directly with participants enrolled in programs such as HealthierMe, providing personalised coaching, education, motivation, and support to help them better manage their health and achieve their goals. 

About the Role 

As a Health Coach, you will deliver evidence-based coaching and behaviour change interventions through phone and digital channels. Using your clinical expertise, you will support individuals living with chronic conditions and health risk factors to build confidence, improve self-management skills, and make sustainable lifestyle changes. 

Reporting to the Health Coach Team Lead, you'll be part of a collaborative multidisciplinary team committed to delivering high-quality virtual healthcare. This is a 12-month fixed-term contract, with the possibility of becoming permanent later on. The role is primarily Monday to Friday, with standard hours of 9:00am to 5:00pm and one rostered late shift each week from 11:00am to 7:00pm (likely on a Monday or Wednesday). This is a hybrid position, requiring attendance in the office 2 to 3 days per week, with the remaining days worked remotely.

Key Responsibilities 

  • Deliver personalised health coaching and behaviour change support to program participants
  • Conduct initial assessments and develop individualised care plans
  • Set meaningful health goals and monitor participant progress
  • Apply motivational interviewing and health coaching techniques to drive engagement and outcomes
  • Identify clinical risks and escalate or refer participants when appropriate
  • Manage a caseload of participants while meeting service and engagement requirements
  • Maintain accurate and timely clinical documentation
  • Collaborate with allied health professionals, nurses and other stakeholders
  • Contribute to continuous improvement initiatives and program development 

Essential Requirements 

  • Tertiary qualification in Physiotherapy, Exercise Physiology, Nursing, Occupational Therapy, or Dietetics.
  • Current unrestricted professional registration.
  • Demonstrated clinical reasoning skills
  • Ability to manage competing priorities in a fast-paced environment 

Desirable 

  • Health coaching or motivational interviewing experience
  • Telehealth or virtual healthcare delivery experience
  • Experience supporting clients with chronic disease and health risk factors
  • Experience working within multidisciplinary clinical teams
