Your new role
A 12-month fixed-term full-time opportunity is now available within our Remedy Healthcare’s Health Coaching team.
In this role you will be responsible for delivering Remedy Healthcare’s chronic disease and risk factor management program, HealthierMe. In this role you will support individuals to achieve health goals and optimise the management of their health using behaviour change techniques and education.
You will also:
- Deliver quality clinical care through appropriate and effective utilisation of health coaching, behaviour change and motivational interviewing skills/principles/techniques
- Adhere to new and review/progress appointment management requirements of your case load
- Escalate and manage clinical deteriorations
- Contribute to a culture of continuous improvement by identifying opportunities for, and implementation of, quality and process improvement
- Participate in process and program revisions and implementation in response to new customers and client groups
About you
- Allied health or nursing qualification – Physiotherapist, Exercise Physiologist, Nurse, Occupational Therapist or Dietitian
- Current, unrestricted registration with appropriate body
- Experience supporting community-dwelling, working age or older individuals to manage their health risk factors
- Previous experience in health coaching/behaviour change motivation desirable but not essential
- Previous experience in telephone/technology-based clinical delivery is desirable
- Previous experience working in a multidisciplinary collaborative team
- Demonstrated ability to work in a dynamic environment within tight timeframes and completing priorities
- Demonstrated excellent clinical reasoning
