As a Clinical Care Partner, you’ll play a vital role in supporting older Australians to live well at home, by guiding care planning, uplifting health literacy, and ensuring safe, quality care.

 

This is a virtual-first role, open to candidates based in QLD, NSW, VIC, or ACT with occasional travel for in-home visits. Due to business growth, we have multiple roles available.

 

In the past twelve years, we’ve provided in-home health care and telehealth for almost 100,000 Australians, helping them manage their health and enjoy life to the full. Our services integrate physical, mental, and social health, and focus on reducing risk. And we know they work, our clients have fewer hospital admissions, shorter hospital stays, and less costly hospital and insurance claims.

 

We’re creating an environment where everyone in the system wins. We believe in a better healthcare system, one where business, government and health care providers work together, hospitals meet demand and people are in control of their health.

 

About the Role

As a Clinical Care Partner, you will:

  • Provide clinical guidance and coaching to Care Partners managing complex customer needs
  • Conduct virtual assessments and co-design health plans that reflect customer goals
  • Advocate for customers in multidisciplinary forums and liaise with GPs, hospitals, and service providers 
  • Monitor care quality, manage risk, and support transitions across care settings
  • Champion reablement and person-centred care, helping customers live vibrant, independent lives
  • Collaborate across teams to grow safe minutes of care and improve health outcomes

 

What You’ll Bring

  • Tertiary qualifications in nursing or allied health, with current registration
  • Minimum 4+ years’ experience in subacute or community care, ideally with ageing populations
  • Strong clinical reasoning and communication skills
  • Experience in hospital discharge planning, care coordination, and multidisciplinary collaboration
  • Tech-savvy and confident using virtual platforms to connect with customers and teams
  • A valid driver’s licence and access to a reliable vehicle (for occasional in-home visits)
