Our Opportunity to you
We are looking for talented Occupational Therapists, Physiotherapists, or Nurses to join our Remedy family. Working as part of our multidisciplinary healthcare team, you’ll coordinate and support the delivery of home-based health care to clients across Australia.
This is a 12-month fixed-term opportunity offering a full-time Monday to Friday schedule with the flexibility of hybrid working arrangements.
We're open to applications from candidates based in Western Australia or across Australia's East Coast, with the option to work from our Delhi Street West Perth office or one of our many East Coast office locations. For candidates based on the East Coast, the role will require availability during Australian Western Standard Time (AWST) business hours to support clients and facilitate collaboration with key stakeholders and teams.

The day to day
- Coordinating and managing Rehab at Home programs for clients discharging from hospital to home-based care following orthopaedic surgery or deconditioning post hospital admission
- Supporting and communicating with clients remotely to achieve their personal and clinical rehabilitation goals within their home environment
- Implementing, monitoring, and modifying in-home care plans to meet client needs
- Managing clinical case escalations and variations to expected clinical progress
- Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers, and funders
- Helping to improve the health and wellbeing of hundreds of Australians

About You
- Hold an Occupational Therapy, Physiotherapy or Nursing degree, along with current unrestricted AHPRA registration
- Have demonstrated experience working in a multidisciplinary collaborative team
- Excellent knowledge of hospital discharge planning
- Ability to work in a dynamic environment with tight timeframes and competing priorities
- Excellent clinical reasoning and problem-solving skills
- Strong administration and time management skills, tech savvy
- Passionate about health and committed to making a real difference in the community
