Clinical Care Partner

We are seeking a Clinical Care Partner to support safe, coordinated and proactive care for customers with complex or changing needs.

This role is a key clinical position within the Neighbourhood Care Partnering model, providing clinical leadership, escalation management and decision support to Care Partners and the broader multidisciplinary team. The Clinical Care Partner plays a critical role in identifying and managing clinical risk, supporting customer outcomes and ensuring customers receive the right care at the right time.

This is a Permanent Full-Time opportunity offering flexibility and a great work-life balance.

• Flexible work hours: 8:30am – 4:30pm or 9:00am – 5:00pm, Monday to Friday
• Remuneration ranges from $105,000 to $120,000 TFEC
• Hybrid work model combining client visits, Branch days (3 days - Dubbo office) & Work from home (2 days)
• Supporting SAH customers

About the role

In this role, you will support customers with higher complexity needs while providing clinical guidance and capability uplift across Care Partnering teams.

You will:

• Provide clinical leadership and escalation support for customers with complex, unstable or changing needs.
• Support Care Partners to identify, assess and respond appropriately to clinical risk and deterioration.
• Undertake clinical reviews and interventions to optimise customer outcomes.
• Apply clinical judgement to support safe and timely decision making.
• Support transitions of care, including hospital admissions, discharges and changes in customer needs.
• Facilitate collaboration across Care Partnering, Service Delivery, Clinical Services and external healthcare providers.
• Assist with clinical governance, risk management and quality improvement activities.
• Contribute to workforce capability through coaching, mentoring and sharing clinical expertise.
• Support proactive care planning and clinical oversight for higher-risk customer cohorts.
• Ensure accurate documentation and appropriate use of clinical systems and escalation pathways.

What you’ll bring

You are a clinically experienced professional who enjoys improving customer outcomes, supporting others and working collaboratively within a multidisciplinary environment.

You will bring:

• Tertiary qualifications in Nursing or Allied Health with current AHPRA registration where applicable.
• Demonstrated 4+ years experience supporting older people in community, subacute or complex care settings.
• Strong clinical assessment, reasoning and decision-making capability.
• Experience managing customer risk and responding to clinical deterioration.
• Ability to coach and support others to build confidence and capability.
• Excellent communication and stakeholder engagement skills.
• Experience working collaboratively across multidisciplinary teams.
• Understanding and adhence to clinical governance, quality and risk frameworks.
• Ability to manage competing priorities in a dynamic environment.
• A customer-centred approach focused on safety, wellbeing and positive outcomes.
