Our Opportunity
We are looking for a telehealth Home Care Package Care Partner to join our Self Managed Division. You will be able to assist the elderly to stay in their own homes with appropriate levels of care and support, whilst enjoying the flexibility and stability of a hybrid telehealth role - no more travelling between clients!

As a Care Partner, you will:
- Deliver telehealth Care Planning, Assessment and Coordination for elderly Australians.
- Manage client service delivery across all levels under the Home Care Package program.
- Develop, monitor, and reassess care plans in consultation with clients, their families and other healthcare professionals.
- Complete telehealth or onsite assessments including OHS assessments.
- Monitor the budget of care packages and ensure clients understand service fees.
- Lead and work collaboratively with our schedulers, clinical team, and support staff.
- Ensure compliance with all relevant legislative and industry standards.
We are looking for someone with:
- Experience in aged care, with knowledge of Home Care Packages and Aged Care Quality Standards.
- Strong computer skills in CRM systems, database management, Microsoft Office.
- Problem-solving abilities with the ability to think critically.
- Strong interpersonal and verbal/written communication skills.
- The ability to work at a fast pace with effective time management skills.
- A strong team focus and the ability to develop rapport with others.
- The willingness to learn new systems, policies and procedures and be challenged by the development of a high growth business.
