Our Opportunity
We are looking for a telehealth Care Partner who speaks fluent Mandarin to join the Self Managed Division Happy Living brand. Happy Living is a national brand with over 2,000 clients mainly from the Chinese community. Mandarin language fluency is essential and Cantonese language fluency is highly desirable.

As a Care Partner, you will:
- Deliver telehealth Care Planning, Assessment and Coordination for elderly Australians in the Chinese community.
- Manage client service delivery across all levels under the Home Care Package program.
- Develop, monitor, and reassess care plans in consultation with clients, their families and other healthcare professionals.
- Complete telehealth or onsite assessments including OHS assessments.
- Monitor the budget of care packages and ensure clients understand service fees.
- Lead and work collaboratively with our schedulers, clinical team, and support staff.
- Ensure compliance with all relevant legislative and industry standards.
We are looking for someone with:
- Fluency in Mandarin is essential. Fluency in Cantonese as well is highly desirable.
- Experience in aged care, with knowledge of Home Care Packages and Aged Care Quality Standards.
- Strong computer skills in CRM systems, database management, Microsoft Office.
- Problem-solving abilities with the ability to think critically.
- Strong interpersonal and verbal/written communication skills.
- The ability to work at a fast pace with effective time management skills.
- A strong team focus and the ability to develop rapport with others.
- The willingness to learn new systems, policies and procedures and be challenged by the development of a high growth business.
