As an Aged Care Case Manager, you will be responsible for coordinating and managing customer care journeys, ensuring services are aligned to individual goals, funding, and clinical needs. You’ll work closely with customers, families, clinical teams, and external providers to develop and manage care plans and ensure a seamless experience across the care journey.

 

This is a hybrid opportunity with the option to work 2 days from home and 3 days from the Clayton Victorian Branch within our business hours of Monday to Friday between 8:30am and 5pm. This is a fully virtual, phone-based role with no in-home or direct care responsibilities.

 

Key Accountabilities

  • Manage a portfolio of customers, acting as their primary point of contact, by building trusted relationships with customers and support them to achieve their goals at home.
  • Conduct virtual onboarding assessments, develop holistic health plans, and coordinate multidisciplinary service aligned to customer goals and funding.
  • Monitor customer progress and adjust care plans as needed to ensure that opportunities are identified to enhance services and improve customer outcomes.
  • Coordinate services with clinical teams and external providers to ensure the delivery of safe, person-centred care and optimal outcomes.
  • Support customers through care transitions including hospital discharge or changes in health needs
  • Maintain accurate records in CRM and enterprise systems, ensuring compliance and audit readiness
  • Contribute to quality, safety, and continuous improvement initiatives

 

What You’ll Bring

    • Experience in case management is essential - aged care is desirable but we are open to other industries such as banking, real estate or workers compensation to name a few!   
    • Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy
    • Strong verbal and written communication skills & tech savvy with Microsoft Suite and CRM systems
    • Knowledge of aged care funding streams, quality standards, audits and care transitions such as discharge planning is desirable
    • High organisation skills coupled with adept problem-solving and decision-making skills
    • Collaborative mindset, able to work effectively in a team-based, matrix environment, liaising with families, clinicians, and external providers.
    • Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
