Your New Role

Come and join our clinical and care team at Rathdowne Place in Carlton , where we are leading the industry in driving our award winning ‘Better Together’ model and raising the quality of life for our residents and their families. 

What does your new role look like?

  • Supporting the Clinical Care Manager and multidisciplinary team to identify and analyse data on identified indicators and areas of risk.
  • Review processes and documentation, and conduct audits, to identify gaps or trends and then implement staff education and continuous quality improvement initiatives in consultation with the Clinical care manager and in line with best practice guidelines, organisational policies, and relevant legislation.
  • Be able to identify AN-ACC rates in correspondence to resident care needs.
  • Support the resident on the journey when they join us.
  • Ensure assessments, care strategies and ongoing documentation to provide the evidence to support AN-ACC funding and track funding for each AN-ACC resident.
  • Provide advice and support to Residential Aged Care staff around funding evidence requirements, work along the teams assisting in the resident assessment process.

 

About you 

As well as being an accomplished Registered Nurse and Leader, you will align to our Values of Bold, Warm and Honest.  You will be able to demonstrate empathy and the ability to provide excellent clinical care for our residents, whilst effectively leading and supporting a team.

  • AHPRA Registration and a bachelor’s degree in nursing
  • Strong ability to identify and implement areas for improvement initiatives to increase compliance levels and client satisfaction.
  • Experience in leading and training Care and Clinical team members whilst creating an engaging and supportive work culture.
  • Proficiency using various systems, including online care management programs.
  • Roster will be Monday, Tuesday , Thursday and Friday 8am to 4pm = 30 hours per week.
  • Fully vaccinated for Influenza and COVID 19.
