Our Opportunity to you

The Community Connect Specialist plays a crucial role in effectively deploying clinical workers to clients in our community.

 

This hybrid role will be working out of our Aspley office in Brisbane and from home on a permanent full time or part time basis. 

 

The day to day

  • Providing quality administrative support to the allied health and nursing clinical teams, including reporting, document processing and client data management.
  • Supporting the clinical workforce through referral processing and allocation.
  • Responsible for document management, customer and referrer liaison, scheduling support of allied health and nursing services, and ongoing client case support throughout our customer’s journey.
  • Management and collaboration with new and existing service providers to deliver care to our clients in their home.

About you

  • Self-motivated medical and office administrator, skilled in customer service
  • Strong administration and time management skills, comfortable with using multiple systems
  • Experience in service delivery allocation and scheduling, along with effective case management and support skills
  • Effective communication skills and building rapport with external stakeholders
  • Adaptable and solution focused with the tenacity to overcome challenges
