About Ramsay Connect

Ramsay Connect is a national provider of home and community-based healthcare and support both in person and utilising technology to enable virtual care delivery. Ramsay Connect works closely with patients, hospitals, and primary care to support the transition of individuals from hospital to home and supporting them to stay well and out of hospital, providing integrated quality healthcare in the home and community setting.

 

Ramsay Connect is a joint venture between Ramsay Health Plus (part of Ramsay Health Care) and Remedy Healthcare (part of Australian Unity), a national provider of in-home healthcare services. Australian Unity would be the employing entity for this role, however the position would be seconded to Ramsay Connect.

 

Our opportunity to you

The Clinical Nurse/Clinical Nurse Specialist (herein referred to as Clinical Nurse) - Cardiac, is responsible for the delivery of quality, client-centred care to individuals accessing the Ramsay Connect Virtual (RCV) Heart Failure service. They will also lead the integration of care pathways in the South East Queensland area. The role will primarily be involved with the RCV Heart Failure service, although it may also involve support of clients within other program areas such as Diabetes programs and other chronic disease programs.

 

Role Details

Term: Fixed-term contract (12 months) with opportunity to extend

FTE: Full-time (part-time will be considered)

Location: Brisbane preferable but would consider broader South East Queensland area if willing to travel. There is flexibility to work from home and out of the Ramsay hospital sites

 

The day to day

  • Working within a digitally enabled model of care to support people in the community with heart failure
  • Effective and responsive admission management, application of evidence-based, technology-enabled clinical care and monitoring
  • Strong integration with hospital and primary care teams and collaboration with a multi-disciplinary heart failure care team
  • Establishing strong working relationships with specialists, hospitals and GPs in the area to grow referral pathways into the service
  • Foster integration with the service through prompt escalation, case conferences and medication titration (as directed by the patient’s specialist or GP)

About You

  • Advanced clinical skills and recent cardiac experience and heart failure management
  • Delivery of care within a multi-disciplinary framework
  • Contribution to continuous improvement and service development activities
  • Experience working with multiple stakeholders to provide appropriate engagement and education
  • Full clean drivers license and ability to commute to hospitals in the Brisbane, Gold Coast and Sunshine coast where required
  • Tech savvy, strong administration and time management skills
  • Strong work ethic and ability to deliver high quality work within short time constraints
  • Registered Nurse with unrestricted AHPRA registration
