Are you a clinically experienced palliative care professional seeking a role where your expertise truly matters? At Australian Unity, we’re looking for a dedicated Clinical Care Partner (CCP) to lead end-of-life care coordination under the Support at Home program. This is a unique opportunity to support older Australians with a life expectancy of three months or less, enabling them to remain at home with dignity, safety, and comfort.

 

About the Role

 

As a CCP in our End-of-Life Portfolio, you’ll provide holistic case management and service coordination for customers accessing short-term EOL funding. You’ll work in partnership with GPs, specialist palliative care services, allied health, nursing teams, and families to deliver seamless, person-centred care aligned with national aged care standards and funding guidelines.

 

What You’ll Do

  • Conduct comprehensive assessments and develop tailored end-of-life care plans
  • Facilitate multidisciplinary case conferences and ensure clinical safety
  • Monitor care delivery and adjust services as needs evolve
  • Manage Support at Home EOL funding and ensure compliance
  • Provide advocacy, education, and psychosocial support to families
  • Lead transitions and contribute to continuous improvement in EOL care

 

What You’ll Bring

  • Tertiary qualifications in allied health or nursing with current registration
  • Postgraduate qualifications in Palliative Care (or equivalent clinical experience)
  • Minimum 5 years’ experience in direct palliative care delivery
  • Expertise in advanced care planning, case management, and multidisciplinary coordination
  • Strong understanding of Support at Home EOL funding and compliance
  • Compassionate communication and motivational interviewing skills
  • Familiarity with ELDAC frameworks and state-funded palliative care models
